Our Testing and Review Process 

To determine the best maternity underwear for pregnancy and the postpartum period, we consulted experts about what to look for in maternity underwear and researched options from well-known brands. Several products on this list were also part of our test of postpartum underwear, conducted by our own staffers who had recently given birth. We paid special attention to each of the following attributes when deciding which pairs belong on our list:  

  • Comfort: We noted how comfortable the undies were in terms of waistband thickness and location, level of compression, and fabric softness.
  • Fit: Keeping butt coverage and thigh hole size in mind, we assessed how well the panties fit and felt.
  • Breathability: Body temperature changes are common in pregnancy as hormones fluctuate, so we assessed how much airflow and moisture-wicking the fabric allowed.
  • Support: We noted if the panties provided light compression to help support a growing belly in pregnancy and a deflating belly postpartum.
  • Care: We preferred machine wash options to make life easier, but noted any specific care instructions so you can ensure your panties last as long as possible.

Factors to Consider When Buying Maternity Underwear

  • Stage of Pregnancy: Your belly will extend roughly five to six times its baseline size over the course of pregnancy. As pelvis ligaments loosen to prepare for baby’s arrival, your hips spread slightly and the bottom widens, often sparking the need for larger-sized undergarments. To avoid purchasing new underwear every six weeks or so, Dr. Pickett suggests choosing pairs with enough elasticity to accommodate this rapid size increase.
  • Comfort and Safety: McKinney-Kirk recommends looking for soft materials that won't dig in or feel restrictive and steering clear of underwear that feels too snug to avoid excessive compression on the belly or hips. “Tight undies can cause discomfort and even affect blood flow,” she warns. Dr. Pickett agrees, adding that pregnant people should avoid underwear that sits on the bikini line, as it may cause chafing and irritation under a growing belly.
  • Size: Choosing the right size for maternity underwear can be tricky, especially when it feels like your body can change dramatically overnight. Most pregnancy underwear corresponds to your pre-pregnancy size, but be sure to check each brand’s size chart.
  • Support: Embrace the “granny panty” look, says Dr. Pickett. “Panties that sit higher on the belly can support the growing tummy by lifting it up from the bottom, which can decrease pain and discomfort in the ligaments,” she says. For a little extra support, McKinney-Kirk also suggests maternity undies with built-in belly panels or wider waistbands.
  • Style: The two main types of maternity underwear are over-belly, which has a stretchy panel that covers and supports your bump with light compression and/or a breathable panel, and under-belly, a design that rests around your hips and underneath your bump that’s ideal during warmer months. Maternity underwear is also available in bikini, boy short, and even thong. (Our experts say thongs can make you more susceptible to vaginal infections, though a cotton gusset can reduce this risk.) Try a few styles throughout your pregnancy since your preferences may change.
  • Material: Dr. Pickett recommends panties with a cotton-lined crotch. “There’s typically extra moisture from leukorrhea (a milky discharge due to rapid cell turnover in the vagina during pregnancy) and sweat, which can increase the risk for overgrowth of yeast and bacteria.” Look for brands with cotton panels and Lycra or spandex throughout the panty to help support the uterus and stabilize the hips.
  • Stretchiness: Opt for styles with give, allowing them to accommodate your changing shape without feeling tight or constricted. Most maternity underwear is made with a bit of stretchy material and has forgiving elastic. 
  • Leakproof Features: Thanks to leukorrhea, leaking is something pregnant women frequently experience. Thankfully, today’s leakproof options feel less like diapers and more like comfortable, stylish choices that keep you dry and confident. You may prefer washable or disposable options.

When should you start wearing maternity underwear?

Much like your regular clothes, when you feel like your everyday undergarments aren’t fitting anymore, it’s time to invest in maternity underwear. As your body continues to undergo changes throughout pregnancy, you may find you prefer different styles.

Does your underwear size change when you’re pregnant?

Typically, you should still wear the same size in maternity underwear as you do in regular underwear. However, some people may want to go up a size to allow more room to grow. Note that each brand is different, so consult the size chart to ensure you get a good fit.

How many pairs of maternity underwear do you need? 

This will differ for each person. Consider how frequently leakage prompts underwear changes throughout the day and how often you want to do laundry, then get as many pairs as you need to have enough between loads. You’ll likely want at least seven pairs. Many options come in three-packs and five-packs to help you save money.  

Can I still wear maternity underwear after the baby is born?

Absolutely. McKinney-Kirk suggests wearing maternity briefs or high-waisted undies to help support your healing from postpartum hemorrhoids, vaginal tears, or a C-section incision to give your delicate areas some breathing room.
